About the role

Key responsibilities & impact
  • Join a new pharmacist-led clinic to deliver comprehensive medication management services
  • Manage GLP-1 therapies, including patient onboarding, medication initiation, titration, and ongoing monitoring under established delegation protocols
  • Provide high-quality, patient-centered care independently in a fast-paced telehealth environment
  • Support the needs of a rapidly growing patient population, with opportunities to expand into additional therapeutic areas
  • Deliver education and training to pharmacy residents and PharmD students through both practical (clinical) and didactic experiences

Requirements

What you’ll need
  • Bachelor's Degree in Pharmacy or a PharmD degree Required
  • Completion of an ASHP Accredited Residency training program Preferred
  • 1 year of clinical patient care and orders management experience Required
  • 3 years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited residency training program Preferred
  • Licensed to practice pharmacy in the State of Wisconsin within 90 Days Required
  • Basic Life Support (BLS) within 90 Days Required
  • Certification through the appropriate certifying program (Board of Pharmacy Specialties, National Board of Nutrition Support Certification, National Certification Board for Anticoagulation Providers, Specialty Pharmacy Certification Board, etc.) Preferred
