Key responsibilities & impact

Join a new pharmacist-led clinic to deliver comprehensive medication management services
Manage GLP-1 therapies, including patient onboarding, medication initiation, titration, and ongoing monitoring under established delegation protocols
Provide high-quality, patient-centered care independently in a fast-paced telehealth environment
Support the needs of a rapidly growing patient population, with opportunities to expand into additional therapeutic areas
Deliver education and training to pharmacy residents and PharmD students through both practical (clinical) and didactic experiences

Requirements

What you’ll need

Bachelor's Degree in Pharmacy or a PharmD degree Required
Completion of an ASHP Accredited Residency training program Preferred
1 year of clinical patient care and orders management experience Required
3 years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited residency training program Preferred
Licensed to practice pharmacy in the State of Wisconsin within 90 Days Required
Basic Life Support (BLS) within 90 Days Required
Certification through the appropriate certifying program (Board of Pharmacy Specialties, National Board of Nutrition Support Certification, National Certification Board for Anticoagulation Providers, Specialty Pharmacy Certification Board, etc.) Preferred
