Provide consultative and rehabilitative treatment services to patients
Work collaboratively with the interdisciplinary team to create comprehensive evidence-based treatment plans to maximize functional outcomes and plan of care

Work within a large, dynamic rehabilitation team to continually provide exceptional patient and family centered care
Schedule: 60% FTE (24 hours per week) Monday through Friday day shift; hours may vary based on operational needs
Requirements
Bachelor's Degree in Physical Therapy Required
Licensure as a Physical Therapist in the State of Wisconsin Upon Hire Required
Basic Life Support (BLS) certification within 3 months Required
2 years of prior relevant experience Preferred
60% FTE (0.6), 24 hours per week; department prefers ability and preference to work 40 hours per week
Availability to work Monday through Friday day shift
Ability to work on-site at the Verona Clinic in Verona, WI
