About the role
- Receives referrals from physicians and mid-level providers.
- Perform initial evaluations of client needs and develop customized treatment plans.
- Provide counseling/talk therapy to scheduled patients utilizing professional counseling standards.
- Formulates differential diagnosis of mild to moderate mental health conditions.
- Formulates short-term and long-term counseling plans for patients under care.
- Consults with medical director and other providers regarding complex patients and regarding prescription medications prescribed by providers.
- Identify patients with urgent mental health conditions (e.g. suicidal, homicidal, psychotic, etc.) and make appropriate referrals to available emergency or psychiatric treatment facilities.
- Provide written document of counseling sessions, assessments, interventions.
- Prepares and conducts group counseling utilizing Cognitive Behavioral Therapy (CBT).
- Establishes systematic follow-up for evaluation of the plan of care and reassesses and modifies the plan, as necessary, including initiation of referrals and consultation with other health care providers as appropriate.
- Assures patient and family teaching is adequate to manage the treatment plan.
- Maintain appropriate documentation of clinical and counseling services, including all necessary billing information.
- Maintain and update professional knowledge and proficiency through continuing education, staff meetings, workshops.
Requirements
- Master’s or doctoral degree in Psychology, Social Work, Counseling, Marriage & Family Therapy, or related field
- Active, unrestricted license: LCSW, LMFT, LPC, or LPCC in Colorado
- Strong background in evidence-based practices (e.g., CBT, DBT, EMDR, ACT)
- Ability to provide therapy independently
- Comfort using an EHR and telehealth platforms with adherance to HIPAA
- Experience conducting psychotherapy with specific populations (children, adolescents, trauma, couples, etc.
- Experience in outpatient or group practice settings
- Demonstrates positive behavior, attitude, and commitment as documented by supervising physician, patient satisfaction surveys, and feedback from other personnel.
- Work with physicians, mid-levels and other members of medical care teams to evaluate patient conditions and develop comprehensive care plans.
- Demonstrates positive teamwork, cooperative efforts, prompt and attentive service to patients and colleagues.
