  • Provide comprehensive waiver case management for individuals with severe mental illness or traumatic brain injury
  • Help clients access essential resources and maintain stability
  • Improve long-term wellbeing of clients
  • Evaluate clients’ needs, limitations, and desires
  • Build rapport with clients and their families
  • Assist clients in receiving services by telephone and in person
  • Respond to crisis situations
  • Monitor clients' situations and follow up on their progress

Requirements

What you’ll need
  • Bachelor’s degree in social work or a closely related field
  • Two years (2,080 annual hours) of providing waiver case management services
  • Valid driver’s license and vehicle available for business use
  • Master’s degree or higher in social work or a closely related field (preferred)
  • Three years or more (2,080 annual hours) of providing waiver case management services (preferred)
  • Experience working within SSIS database (preferred)
  • Strong verbal and written communication skills (preferred)
  • Skilled in problem-solving, critical thinking, and effective time management (preferred)
