Provide comprehensive waiver case management for individuals with severe mental illness or traumatic brain injury
Help clients access essential resources and maintain stability
Improve long-term wellbeing of clients
Evaluate clients’ needs, limitations, and desires
Build rapport with clients and their families
Assist clients in receiving services by telephone and in person

Requirements

What you’ll need

Bachelor’s degree in social work or a closely related field
Two years (2,080 annual hours) of providing waiver case management services
Valid driver’s license and vehicle available for business use
Master’s degree or higher in social work or a closely related field (preferred)
Three years or more (2,080 annual hours) of providing waiver case management services (preferred)
Experience working within SSIS database (preferred)
Strong verbal and written communication skills (preferred)
Skilled in problem-solving, critical thinking, and effective time management (preferred)
