About the role
Key responsibilities & impact- Completes clinical mental health screenings, CFTs, and PCPs
- Provides supervised behavioral health support services to individuals, families, and groups
- Assesses progress toward goals of individuals served, coordinating supports and resources, as needed
- Prepares and submits documentation of services within 24 hours of service delivery
- Assists with crisis response and care coordination under supervision as indicated
- Consults regularly with clinical team members on member care
- Communicates with Behavioral Health colleagues, Primary Care providers, and other providers and agencies as needed to facilitate integrative care
- Participates in staff development meetings and in quality of care activities on a regular basis
- Performs other related duties as assigned
Requirements
What you’ll need- Strong therapy, prescreening, and clinical interviewing skills
- 1 year of experience providing urgent care or crisis intervention required
- 2 year of experience providing behavioral health clinical interventions
- Experience in completing Mental Health Screenings
- Treatment Planning / Person-Centered-Planning Expertise
- Efficient & Effective Practice Management in a Clinical Setting
- Client Focus & Strong Ability to Function as a Team Member
- Strong Individual, Family, & Group Support Skills
- Flexibility with willingness to work in office, client home, and/or community
