Key responsibilities & impact

Collaborates with the Regional Director of Nursing and the Pharmacy Team to facilitate and oversee nursing care coordination.
Review Intakes, draft referrals, and secure nursing care for patients with experienced RN's in the Home or Infusion Center setting.
Communicate effectively with patients and nurses to provide safe and quality services.
Organize, maintain, and communicate patient care information/documents to internal and external staff.
Ensure clear and concise documentation within EMR systems.
Ensure compliance with external standards and regulations.
Serve as a liaison between interdisciplinary teams.
Set up and assist in maintaining RN work schedules to ensure staffing requirements are met.
Maintain daily staffing reports and provide weekly updates to management.
Perform other duties as assigned.

Requirements

What you’ll need

High school diploma or equivalent.
Experience as a staffing/scheduling coordinator or in a similar role.
General knowledge of medical terminology.
Access to reliable high-speed internet connection and a dedicated workspace.
