Health Services Coordinator

About the role
Key responsibilities & impact- Serve as primary point of contact for newly referred patients
- Conduct outbound outreach to explain Remote Patient Monitoring programs
- Engage patients telephonically to establish rapport and encourage participation
- Document patient information in the electronic health record (EHR)
- Collaborate with nurse care managers to support patient care goals
- Respond to inbound inquiries and initiate communications with patients and providers
Requirements
What you’ll need- Completion of a Medical Assisting program accredited by CAAHEP or ABHES
- At least six (6) months of experience performing general duties as a Medical Assistant or three (3) years experience working in a clinic or comparable healthcare setting with direct patient contact
- Knowledge of medical terminology and prior experience using electronic health record (EHR) systems
- Bilingual (Spanish) preferred
- Active CNA certification or equivalent certification (e.g., Pharmacy Technician), in good standing
