Key responsibilities & impact

Serve as primary point of contact for newly referred patients
Conduct outbound outreach to explain Remote Patient Monitoring programs
Engage patients telephonically to establish rapport and encourage participation
Document patient information in the electronic health record (EHR)
Collaborate with nurse care managers to support patient care goals
Respond to inbound inquiries and initiate communications with patients and providers

Requirements

What you’ll need

Completion of a Medical Assisting program accredited by CAAHEP or ABHES
At least six (6) months of experience performing general duties as a Medical Assistant or three (3) years experience working in a clinic or comparable healthcare setting with direct patient contact
Knowledge of medical terminology and prior experience using electronic health record (EHR) systems
Bilingual (Spanish) preferred
Active CNA certification or equivalent certification (e.g., Pharmacy Technician), in good standing
