About the role

Key responsibilities & impact
  • Conduct outbound outreach to newly referred prospective patients and explain Remote Patient Monitoring programs, services, benefits, expectations, and value
  • Engage patients by telephone to establish rapport, answer questions, and encourage participation and adherence
  • Complete Welcome Calls and Post-Physician Visit Updates
  • Document patient information accurately in the electronic health record (EHR)
  • Collaborate with nurse care managers and the broader clinical care team to support patient care goals
  • Track hospitalizations and notify nursing staff upon patient discharge
  • Coordinate patient assistance services such as transportation, housekeeping, home modifications, and food assistance
  • Follow up with patients to confirm services are initiated and meeting expectations
  • Respond to inbound patient, client, and provider inquiries and determine service needs
  • Communicate with patients, clients, physician offices, and care providers by phone, fax, and email
  • Maintain accurate provider contact information
  • Escalate unresolved patient issues and care-coordination concerns to leadership
  • Meet or exceed call-volume, disengagement-threshold, service-level agreement, and other performance metrics
  • Comply with company policies, HIPAA requirements, and applicable regulations

Requirements

What you’ll need
  • Ability to work independently in a remote environment while maintaining accountability
  • Strong organizational, time-management, multitasking, problem-solving, and critical-thinking skills
  • Excellent verbal and written communication skills with attention to detail
  • Ability to actively listen, demonstrate empathy, build rapport, and maintain productive telephonic relationships
  • Comfort navigating multiple systems and technologies simultaneously
  • Bilingual Spanish preferred
  • Completion of a Medical Assisting program accredited by CAAHEP or ABHES preferred
  • At least six months of experience performing general Medical Assistant duties, or three years of experience in a clinic or comparable healthcare setting with direct patient contact
  • Knowledge of medical terminology
  • Prior experience using electronic health record (EHR) systems
  • Strong technical aptitude and ability to assist patients remotely with health devices and troubleshoot basic issues
  • Ability to type a minimum of 50–60 words per minute while speaking with patients by phone
  • Ability to work schedules aligned to client-specific time zones
  • Active CNA certification or equivalent certification, such as Pharmacy Technician certification, in good standing
  • Ability to work effectively in a fully remote environment
  • Must maintain a dedicated private, quiet, secure, distraction-free home office
  • Reliable internet access meeting company minimum technical requirements
  • Ability to perform extended computer and telephone use in a primarily sedentary role
