About the role

Key responsibilities & impact

Conduct thorough assessments of patients' physical conditions, mobility, and functional abilities to identify impairments and develop appropriate treatment plans.
Collaborate with patients, their families, and other healthcare professionals to establish individualized treatment goals and develop comprehensive plans.
Provide hands-on physical therapy interventions, including therapeutic exercises, gait training, and balance training.
Educate patients and their families on safe and effective therapeutic exercises and pain management strategies.
Maintain accurate and detailed documentation of patient assessments and treatment plans.
Collaborate effectively with other healthcare professionals to ensure coordinated care.

Requirements

What you’ll need

Minimum 2 years experience in a home health
Bilingual a plus
Proficient with Kinnser/Wellsky EHR and OASIS procedures and charting process.
Current Texas state licensure or eligibility to practice as a Physical Therapist.
Bachelor's or Master's or DPT degree in Physical Therapy from an accredited institution.
Valid CPR BLS certification
Excellent interpersonal and communication skills to establish rapport with patients, families, and other healthcare professionals.
Ability to work independently, manage time efficiently, and prioritize tasks effectively.
Proficiency in documenting patient information accurately and maintaining electronic health records.
