About the role

Key responsibilities & impact
  • Provide administrative oversight and supervision of direct reports at the assigned PACE site, including master-level social workers and other PACE team members as needed
  • Promote collaboration and individualized care planning among interdisciplinary team members
  • Coordinate routine assessments and care plans
  • Assist with family conferences and community referrals
  • Monitor team-member documentation
  • Onboard and orient new hires
  • Conduct routine competency and work-performance evaluations
  • Provide guidance on participants’ behavioral health concerns using a person-centered approach
  • Provide guidance related to advanced life planning and end-of-life care
  • Apply state and federal regulatory knowledge to oversee social-work tasks and ensure compliance
  • Collaborate with site leaders on site safety, staffing changes, and program budgets
  • Provide coverage of social-work department roles as needed

Requirements

What you’ll need
  • Master's degree in social work (MSW) from an accredited university required
  • Minimum 2 years' experience in a supervisory position required in a health-related or human services field
  • Minimum 2 years of experience in case management, social advocacy, or care planning with the geriatric population preferred
  • LCSW preferred
  • Bilingual Spanish/English, Chinese/English, or another second language strongly preferred
  • Valid BLS certification or higher required, following American Heart Association (AHA) or American Red Cross guidelines
