Key responsibilities & impact

Supports care coordination by closely working with the community healthcare delivery system.
Facilitates seamless communication and collaboration between healthcare providers, social services, and the community.
Ensures members receive comprehensive, coordinated care.
Helps streamline the delivery of healthcare services, social determinants of health, and resources to the members.
Contributes to improved health outcomes and overall well-being within the community.
Promotes effective care coordination and enhances the overall healthcare experience for the members.

Requirements

What you’ll need

A High School Diploma or GED is required.
A minimum of 3 years of experience directly working in the health care environment or a community outreach setting is required.
Community Health Worker (CHW) certificate.
Preferred experience in a managed health care environment, preferably IPA, HMO, or Health Plan.
Preferred experience working with a diverse population.
Bilingual English/Spanish is strongly preferred.
