About the role
Key responsibilities & impact- Devise personalised support plans to address challenges and enhance older people’s quality of life
- Provide access to ALONE’s services and coordinate access to other community services
- Advocate for older people’s needs within health, social care, and welfare services
- Establish and nurture referral relationships with statutory and non-statutory agencies
- Develop and maintain efficient referral pathways for Support Coordination services
- Adhere to ALONE’s quality management systems and policies
- Monitor and manage KPIs and SMART targets
- Deliver timely reports on service developments, challenges, and achievements
- Manage the distribution and installation of technology supporting older people’s wellbeing and safety
- Maintain records in ALONE’s CRM database
- Travel within the community to coordinate field-based services
Requirements
What you’ll need- Passion for improving older people’s lives and commitment to ALONE’s mission
- Problem-solving skills for needs assessment and resource allocation
- Excellent written and verbal communication skills
- Ability to foster partnerships and negotiate with diverse stakeholders
- Proficiency with technology and data management systems
- Good organisational and multitasking skills
- Solution-focused approach to complex challenges
- Keen attention to detail and accurate record-keeping
- Enthusiasm for setting and achieving SMART targets
- Self-starter able to work well with stakeholders
- Full clean driving licence
- Own transport/car essential
- Willingness and ability to travel within the community
- Entry-level role; background in older people-related fields is not required
