About the role
Key responsibilities & impact- Devise personalised support plans to address challenges and improve older people’s quality of life
- Provide access to ALONE’s services and coordinate access to other community services
- Advocate for older people within health, social care, and welfare services
- Establish and nurture referral relationships with statutory and non-statutory agencies
- Develop and maintain referral pathways for ALONE’s Support Coordination services
- Adhere to ALONE’s quality management systems and policies
- Monitor and manage KPIs and work toward SMART targets
- Deliver timely reports on service developments, challenges, and achievements
- Manage distribution and installation of technology that enhances older people’s wellbeing and safety
- Maintain records in ALONE’s CRM database
Requirements
What you’ll need- Demonstrated passion for improving the lives of older people and commitment to ALONE’s mission
- Problem-solving skills for needs assessment and resource allocation
- Excellent written and verbal communication abilities
- Ability to foster partnerships and negotiate with diverse stakeholders
- Proficiency with technology and data management systems
- Good organisational and multitasking skills
- Solution-focused approach to complex challenges
- Keen attention to detail and accurate record-keeping ability
- Enthusiasm for setting and achieving SMART targets
- Self-starter able to work well with stakeholders
- Full clean driving licence required
- Own car/transport required
- Willingness and ability to travel within the community
- Entry-level applicants accepted; background in older people-related fields not required
