Medical and Eldercare Advocate

About the role
Key responsibilities & impact- Assist clients in navigating healthcare, elder care, and community support systems
- Provide education and guidance regarding healthcare services, benefits, care options, and available resources
- Support clients and family members in understanding healthcare processes and advocating for their own care needs
- Research and identify appropriate resources, referrals, and support services based on individual client circumstances
- Maintain accurate documentation of client interactions and resource recommendations
- Communicate effectively with clients, healthcare providers, community agencies, and other stakeholders while adhering to confidentiality requirements
- Collaborate with internal teams and external organizations as needed to support client outcomes
Requirements
What you’ll need- Bachelor’s degree in Social Work (BSW) or a minimum of five years of related experience in hospital systems, hospice care, geriatric care management, or a closely related field
- Strong understanding of healthcare systems and care coordination processes
- Working knowledge of HIPAA regulations, third-party payer systems, CPT codes, and healthcare documentation requirements
- Excellent verbal communication, active listening, and client engagement skills
- Strong organizational and time-management abilities
- Proficiency with remote communication technologies and standard office software applications
