About the role

Key responsibilities & impact
  • Determine system eligibility and provide resources and reliable information regarding the ODS system and community supports
  • Refer individuals and families to other system services as appropriate
  • Support informed choice of service providers, including Supports Coordination Organizations
  • Conduct assessment and resource mapping using Charting the Life Course planning tools while upholding Self Determination principles
  • Serve as a resource for individuals and families with multiple needs and assist with triage
  • Locate existing and new community resources and document them in individual records
  • Follow up with individual teams to assess resource effectiveness
  • Provide resource consultation for Base funded requests
  • Provide information and support to Supports Coordination Organizations during intake
  • Coordinate with ODS teams including Multi-System, LeCSI, and Planning Team
  • Collaborate with the ODS Planning Team to plan waiver services as necessary
  • Build relationships with community organizations and maintain a working relationship with the PA Family Network
  • Ensure individuals and families receive accurate information about ODS services and providers
  • Assist with Medical Assistance applications when appropriate
  • Explain rights regarding County Conference, Mediation, and Fair Hearing appeals processes
  • Review and analyze county and state reports related to quality, planning, and service delivery
  • Participate in Individual Support Planning team meetings as appropriate
  • Review, revise, and improve county policies relevant to eligibility, planning, and service delivery
  • Implement policies and procedures based on county, state, and federal regulations
  • Perform additional tasks and duties as assigned

Requirements

What you’ll need
  • Bachelor's degree in social work or related field
  • 1 year of previous experience working with individuals/families in intellectual disability, autism, developmental disability, and/or medically complex conditions
  • Or an equivalent combination of education and experience meeting the required knowledge, skills, and abilities
  • Act 33, 34, and FBI clearances
  • Valid driver's license and access to a reliable vehicle
  • Excellent oral and written communication skills
  • Excellent organizational, problem-solving, and analytical skills
  • Excellent knowledge of county, state, and provider delivery systems
  • Basic computer skills in Microsoft Word, Outlook, Excel, internet research methods, Microsoft Teams, and Zoom
  • Proficiency with discovery and navigation, connecting and networking, and goods and services
  • Knowledge of ID, Autism, Developmental Disability, and Medically Complex Condition eligibility criteria and processes
  • Knowledge of DHS service systems including OBH, CHC, and OLTL
  • Ability to connect with professionals to assist with eligibility/registration information and referrals
  • Ability to perform essential duties satisfactorily, with reasonable accommodations as applicable
