About the role
- Ensure a smooth transition for members after hospital discharge
- Coordinate care, provide resources, and educate members/families about post-discharge care
- Create personalized post-discharge care plans
- Educate members and families about medications, appointments, and care instructions
- Collaborate with hospitals and home health agencies
- Maintain accurate documentation of all care coordination activities
Requirements
- Licensed RN in California (required)
- 2+ years of care management experience (required)
- Strong communication, time management, and problem-solving skills
- Comfortable working independently in a fast-paced, remote environment
- Willing to obtain licensure in additional states (company reimburses fees)
- Preferred: Case Management Certification (CCM or equivalent)
- Knowledge of HEDIS and Risk Adjustment programs
