About the role
Key responsibilities & impact- Deliver client-centered aging life care services in the field
- Conduct client visits in homes, assisted living facilities, skilled nursing facilities, physician offices, hospitals, and other care settings
- Assess physical, cognitive, emotional, environmental, social, medication, and care-plan needs
- Complete same-day Daily Logs and visit notes in Excel and MyJunna, including billing entries
- Check pill boxes and attach required photographs of pill boxes, living spaces, vitals whiteboards, meals, and pertinent findings
- Document observations, concerns, communications, follow-up actions, and billable time
- Prepare and adjust weekly client schedules in MyJunna and Outlook
- Check in with the supervisor at the beginning and end of each workday
- Communicate weekly with client families and as needed with physicians, aides, facility staff, and other care team members
- Identify and promptly escalate changes in condition, safety risks, behavioral shifts, care-plan gaps, and billing discrepancies
- Coordinate services across home health aides, physicians, specialists, pharmacies, and community resources
- Ensure safe, documented, and followed-up transitions between care settings
- Participate in care conferences, family meetings, and case reviews
- Provide rotating after-hours and on-call coverage
- Complete formal client hand-offs when taking scheduled time off
- Maintain HIPAA, confidentiality, device, data, operational, and professional compliance
- Attend community events, referral meetings, and networking activities as assigned
- Assist with intake documentation, client onboarding, special projects, SOP documentation, or training content as needed
Requirements
What you’ll need- Minimum of 3 years of experience in care management, eldercare, hospice, social work, or complex senior care coordination
- RN strongly preferred; qualified LPN with 10+ years of eldercare experience, MSW, or CMC credential considered based on demonstrated care management expertise
- Ability to assess older adults with cognitive and physical complexity
- Ability to communicate with families across difficult situations
- Ability to coordinate services across multiple providers and settings
- Strong written communication skills
- Ability to produce accurate, clear, same-day clinical and billing documentation
- Comfort with MyJunna / Care Complete, Microsoft Outlook, Google Sheets, and standard mobile applications
- Valid Florida driver’s license
- Reliable personal vehicle and current personal auto insurance
- Ability to travel locally between multiple client locations each day throughout Palm Beach County
- Dedicated home office with reliable internet and secure computer
- Ability to handle protected health information in full compliance with HIPAA
- CMC credential or active pursuit of certification through NACCM preferred
- Familiarity with Aging Life Care Professional® standards and ALCA ethical guidelines preferred
- Experience with dementia, Alzheimer’s disease, or other cognitive impairment preferred
- Familiarity with assisted living, memory care, and skilled nursing options in Palm Beach County preferred
- Clinical knowledge of older-adult assessment, medication awareness, and care-plan development and monitoring
- Ability to manage multiple clients and competing deadlines
- Ability to maintain professional boundaries
- Clear, timely written and verbal communication
- Ability to receive direction, feedback, and correction and adjust quickly
- Consistent and accurate use of smartphones, tablets, and documentation software
- Ability to work rotating after-hours on-call coverage, including evenings and weekends
- Ability to work in varied home and facility environments, including stairs and uneven surfaces
- Sufficient visual and hearing acuity for clinical assessments and medical documentation
- Florida Level 2 background screening clearance through the AHCA Care Provider Background Screening Clearinghouse
- Employment contingent upon successful background-screening clearance
