About the role

Key responsibilities & impact

Preparation of health & wellness assessment and recommendations
Planning, coordinating and implementing the care plan for each client
Supervise recommendations from the assessment and Care Management meetings
Coordinate client’s physician, dental and other medical appointments
Supervise medical issues including medications and be present at doctor visits
Communicate with and support family members and other responsible parties
Prepare weekly schedules of planned and completed activities - Chart all pertinent client information
Advocacy; obtain restraining orders, testify in circuit court and at the State’s attorney level, testify as an expert witness
Must have reliable transportation to and from the client’s residence
Take potential client intake calls to determine needs, align them with appropriate services and complete paperwork to start services.
Attract referrals and inquiries to build care management services.

Requirements

What you’ll need

Masters in Social Work or RN, CMC Preferred
Eligible for certification by the National Academy of Certified Care Managers
Require at least 5 years of RN experience in eldercare, LPN must have at least 10 years of experience in eldercare
All must have 2 years of experience in Care Management, Social Work or Hospice is preferred.
Strong written and oral communication skills.
Experience working with individuals with Alzheimer's disease or forms of dementia.
Experience with Jewel Code Care Complete or similar Care Management Programs preferred
