💰 $27 - $35 per hour

About the role

  • Preparation of health & wellness assessment and recommendations
  • Planning, coordinating and implementing the care plan for each client
  • Supervise recommendations from the assessment and Care Management meetings
  • Coordinate client’s physician, dental and other medical appointments
  • Supervise medical issues including medications and be present at doctor visits
  • Communicate with and support family members and other responsible parties
  • Prepare weekly schedules of planned and completed activities - Chart all pertinent client information
  • Advocacy; obtain restraining orders, testify in circuit court and at the State’s attorney level, testify as an expert witness
  • Must have reliable transportation to and from the client’s residence
  • Take potential client intake calls to determine needs, align them with appropriate services and complete paperwork to start services.
  • Attract referrals and inquiries to build care management services.

Requirements

  • Masters in Social Work or RN, CMC Preferred
  • Eligible for certification by the National Academy of Certified Care Managers
  • Require at least 5 years of RN experience in eldercare, LPN must have at least 10 years of experience in eldercare
  • All must have 2 years of experience in Care Management, Social Work or Hospice is preferred.
  • Strong written and oral communication skills.
  • Experience working with individuals with Alzheimer's disease or forms of dementia.
  • Experience with Jewel Code Care Complete or similar Care Management Programs preferred
