About the role
Key responsibilities & impact- Preparation of health & wellness assessment and recommendations
- Planning, coordinating and implementing the care plan for each client
- Supervise recommendations from the assessment and Care Management meetings
- Coordinate client’s physician, dental and other medical appointments
- Supervise medical issues including medications and be present at doctor visits
- Communicate with and support family members and other responsible parties
- Prepare weekly schedules of planned and completed activities - Chart all pertinent client information
- Advocacy; obtain restraining orders, testify in circuit court and at the State’s attorney level, testify as an expert witness
- Must have reliable transportation to and from the client’s residence
- Take potential client intake calls to determine needs, align them with appropriate services and complete paperwork to start services.
- Attract referrals and inquiries to build care management services.
Requirements
What you’ll need- Masters in Social Work or RN, CMC Preferred
- Eligible for certification by the National Academy of Certified Care Managers
- Require at least 5 years of RN experience in eldercare, LPN must have at least 10 years of experience in eldercare
- All must have 2 years of experience in Care Management, Social Work or Hospice is preferred.
- Strong written and oral communication skills.
- Experience working with individuals with Alzheimer's disease or forms of dementia.
- Experience with Jewel Code Care Complete or similar Care Management Programs preferred
