About the role

Key responsibilities & impact

Provide direct face-to-face patient care, progressive clinical pharmacy practice; med optimization supported by collaborative practice agreement (extension of primary care and/or specialist)
Manage chronic disease patient panel with focus on full and shared risk, high risk patients with uncontrolled chronic disease states
Oversee pharmaceutical care,Retrieve and evaluate biomedical literature and design medication therapy regimens
Collaborate with the healthcare team to manage formulary adherence and medication use process

Requirements

What you’ll need

PGY1 residency minimum expectation, PGY2 ambulatory or cardio strongly preferred
Board certification must be obtained within one year of role (BCPS or BCACP, etc)
Licensed in the state(s) of practice or eligible for reciprocity required
Immunization and non-vaccine injectable certification required based on area of practice
Bachelor's or Doctorate degree in pharmacy from an ACPE-accredited school of pharmacy required, or an FPGEC certificate from NABP
