About the role
- Interprets physician referrals and completes evaluations utilizing appropriate methods
- Establishes a plan of care and implements treatment based on evaluation results and functional goals
- Monitors and evaluates outcomes and adjusts treatment plan based on patient's response
- Completes patient discharge including providing written home programs, patient/family teaching
- Completes all required documentation following department policy
- Responsible for cleaning up after patient treatment and keeping area neat
- Demonstrates skill in use of modalities and exercise equipment
- Participates in performance improvement activities
Requirements
- Bachelor's Degree in Physical Therapy
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months
- Physical Therapist license issued by the state (Wisconsin or Illinois)
- Intermediate computer skills
- Excellent communication and interpersonal skills
- Demonstrated organization, decision-making, and problem solving skills
