About the role
Key responsibilities & impact- Complete comprehensive evaluations and identify functional limitations (OTs)
- Develop individualized plans of care and measurable treatment goals (OTs)
- Assess ADLs, cognition, strength, coordination, safety, and home environment (OTs)
- Reassess patients and modify treatment plans as clinically appropriate (OTs)
- Recommend appropriate adaptive equipment and home modifications (OTs)
- Provide skilled occupational therapy in accordance with physician orders and CMS Conditions of Participation
- Provide ADL/IADL retraining, therapeutic exercise, functional mobility, balance, coordination, and upper-extremity interventions
- Educate patients and caregivers on fall prevention, energy conservation, adaptive techniques, and home safety
- Train patients in the safe use of adaptive equipment and durable medical equipment
- Maintain timely and accurate documentation
- Collaborate with physicians, caregivers, and the interdisciplinary team to support successful patient outcomes
Requirements
What you’ll need- Degree from an accredited OT or OTA program
- Current Michigan OT or OTA license in good standing
- COTA certification, as applicable
- Home health experience preferred
- Strong clinical judgment and problem-solving skills
- Excellent communication and patient education skills
- Ability to work independently while remaining connected to the care team
- Reliable transportation, valid driver's license, and current auto insurance
- Familiarity with Medicare/CMS documentation preferred
