About the role
Key responsibilities & impact- Complete comprehensive speech-language evaluations to assess communication, cognition, voice, and swallowing needs
- Develop individualized plans of care and measurable treatment goals
- Provide evidence-based interventions for speech, language, cognitive-communication, and swallowing disorders
- Assess and treat dysphagia, aphasia, dysarthria, cognitive impairment, and other communication disorders
- Reassess patient progress and modify treatment plans as clinically appropriate
- Provide skilled speech therapy services in accordance with physician orders and CMS Conditions of Participation (CoPs)
- Educate patients and caregivers on communication strategies, swallowing precautions, aspiration prevention, cognitive techniques, adaptive tools, and individualized Home Exercise Programs (HEPs)
- Maintain timely, accurate documentation in accordance with CMS, payer, and agency requirements
- Collaborate with physicians, caregivers, nursing, therapy, and the interdisciplinary team to support successful patient outcomes
Requirements
What you’ll need- Degree from an accredited Speech-Language Pathology program
- Current Michigan Speech-Language Pathologist license in good standing
- Certificate of Clinical Competence (CCC-SLP) preferred, as applicable
- Home health experience preferred
- Strong clinical judgment, critical-thinking, and problem-solving skills
- Excellent communication, interpersonal, and patient education skills
- Ability to work independently while remaining connected to the interdisciplinary care team
- Reliable transportation, valid driver's license, and current auto insurance
- Familiarity with Medicare/CMS documentation preferred
