About the role

Key responsibilities & impact
  • Complete comprehensive speech-language evaluations to assess communication, cognition, voice, and swallowing needs
  • Develop individualized plans of care and measurable treatment goals
  • Provide evidence-based interventions for speech, language, cognitive-communication, and swallowing disorders
  • Assess and treat dysphagia, aphasia, dysarthria, cognitive impairment, and other communication disorders
  • Reassess patient progress and modify treatment plans as clinically appropriate
  • Provide skilled speech therapy services in accordance with physician orders and CMS Conditions of Participation (CoPs)
  • Educate patients and caregivers on communication strategies, swallowing precautions, aspiration prevention, cognitive techniques, adaptive tools, and individualized Home Exercise Programs (HEPs)
  • Maintain timely, accurate documentation in accordance with CMS, payer, and agency requirements
  • Collaborate with physicians, caregivers, nursing, therapy, and the interdisciplinary team to support successful patient outcomes

Requirements

What you’ll need
  • Degree from an accredited Speech-Language Pathology program
  • Current Michigan Speech-Language Pathologist license in good standing
  • Certificate of Clinical Competence (CCC-SLP) preferred, as applicable
  • Home health experience preferred
  • Strong clinical judgment, critical-thinking, and problem-solving skills
  • Excellent communication, interpersonal, and patient education skills
  • Ability to work independently while remaining connected to the interdisciplinary care team
  • Reliable transportation, valid driver's license, and current auto insurance
  • Familiarity with Medicare/CMS documentation preferred
