About the role
- Conduct on-site client evaluations and develop service plans in conjunction with the authorization
- Perform other field visits as assigned
- Complete visits as scheduled and report daily to scheduler
- Recruit, hire, train, and supervise attendants
- Complete appropriate forms and attach them to the daily visit report
- Maintain supervisor files
- Participate in staff meetings and in-services
- Maintain confidentiality of clients' attendance and staff information
Requirements
- Two years full time study at an accredited college or university
- Individuals hired with a high school diploma or equivalent may substitute one year of full-time employment in a supervisory capacity in a healthcare facility agency or community-based agency for each required year of college
- PHC, MC experience preferred
- Requires full COVID-19 vaccination
