About the role
Key responsibilities & impact- Deliver skilled PTA visits in patients’ homes under the supervising PT’s plan of care
- Execute treatment plans focused on functional mobility and measurable progress, including gait/balance, transfers, strengthening, home safety, and fall prevention
- Set and track objective progress within PTA scope and communicate updates and concerns to the supervising PT
- Coordinate with the multidisciplinary care team by escalating changes in condition and aligning on orders
- Coach patients and caregivers on practical home programs
- Document promptly and thoroughly in Homecare Homebase (HCHB)
Requirements
What you’ll need- Active Texas PTA license in good standing
- Strong clinical judgment and comfort working independently in the field within PTA scope and under PT supervision
- Ability to escalate changes in condition to PT/RN/MD and document rationale clearly
- Strong communication and documentation skills
- Reliable follow-through on treatment plans, visit cadence, and patient goals
- Current driver’s license and comfort with daily travel across assigned territory
- Ability to carry/transport basic therapy equipment as needed
- Competency in core home health PTA scope
- Safety-first mobility skills, including transfers, gait training, balance/fall prevention, and assistive device education
- Strengthening, functional mobility, and home safety assessment/modification education
- Patient/caregiver coaching with practical home programs
- Prior home health experience preferred
- Homecare Homebase (HCHB) or similar home health EMR experience preferred
- Deeper clinical experience in high-signal ortho/neuro populations preferred
- Strong organization and time management preferred
- Willingness to submit to a background check and drug screen
- Legal authorization to work in the country of application
