About the role
Key responsibilities & impact- Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes
- Complete comprehensive in-home assessments and deliver skilled PT visits
- Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention
- Set measurable goals, track objective progress, and adjust treatment quickly when patients stall
- Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving
- Manage treatment and outcomes directly, including PTA supervision where appropriate
- Coach patients and caregivers with practical home programs that actually get done
- Document promptly and thoroughly in Adaptive's proprietary AI-native application
Requirements
What you’ll need- Active Texas PT license (in good standing)
- Strong clinical judgment and comfort working independently in the field
- Comfortable escalating changes in condition to RN/MD and documenting rationale clearly
- Strong communication and documentation
- Reliable follow-through on orders, frequencies, and patient goals
- Current driver’s license and comfort with daily travel across assigned territory
- Able to carry/transport basic therapy equipment as needed
- Competency in core home health PT scope
- Safety-first mobility work: transfers, gait training, balance/fall prevention
- Patient/caregiver coaching with practical home programs
