About the role
Key responsibilities & impact- Conduct scheduled and unscheduled in-person drive-bys on potential home health patients who are non-responsive to phone contact attempts
- Facilitate and support start-of-care (SOC) processes by confirming patient presence, availability, and readiness for initial evaluation
- Serve as a liaison between the patient and the home health care team
- Assist in verifying patient demographic and insurance information
- Communicate findings in real time to the SOC Operations Team and others
- Perform basic assessments within EMT-B scope of practice
- Coordinate with Clinical Manager when a patient is found in acute distress
Requirements
What you’ll need- Current, active Texas EMT-Basic certification
- Valid Texas Texas Driver's License with Valid Auto Insurance
- Reliable personal or agency-provided vehicle
- Strong written and verbal communication skills
- Ability to work independently with minimal supervision
