About the role

Key responsibilities & impact
  • Conduct scheduled and unscheduled in-person drive-bys on potential home health patients who are non-responsive to phone contact attempts
  • Facilitate and support start-of-care (SOC) processes by confirming patient presence, availability, and readiness for initial evaluation
  • Serve as a liaison between the patient and the home health care team
  • Assist in verifying patient demographic and insurance information
  • Communicate findings in real time to the SOC Operations Team and others
  • Perform basic assessments within EMT-B scope of practice
  • Coordinate with Clinical Manager when a patient is found in acute distress

Requirements

What you’ll need
  • Current, active Texas EMT-Basic certification
  • Valid Texas Texas Driver's License with Valid Auto Insurance
  • Reliable personal or agency-provided vehicle
  • Strong written and verbal communication skills
  • Ability to work independently with minimal supervision
