About the role

  • Conduct comprehensive assessments of patients’ health status and needs.
  • Develop and implement individualized care plans in collaboration with healthcare providers.
  • Monitor patients’ progress and adjust care plans as necessary.
  • Provide education and support to patients and their families regarding treatment options and health management.
  • Document all patient interactions and updates in the electronic health record system (Epic).
  • Participate in interdisciplinary team meetings to discuss patient cases and improve care strategies.
  • Advocate for patients’ needs within the healthcare system.
  • Ensure compliance with all regulatory requirements and best practices in patient care.

Requirements

  • Current Registered Nurse (RN) license in the state of practice required.
  • 2 years Acute Care Case Management/Discharge Planning experience.
  • Strong clinical knowledge in behavioral health, emergency medicine, primary care, hospital medicine, medical-surgical nursing, and emergency room protocols.
  • Proficient in taking vital signs and performing basic nursing assessments.
  • Excellent communication skills and ability to work effectively within a team environment.
  • Strong organizational skills with attention to detail in documentation and reporting.
  • Ability to manage multiple priorities while maintaining a high standard of patient care.
  • Charting System: Epic.
  • Travel Experience Required: Yes.
