About the role
- Coordinate patient care and ensure individuals receive necessary medical services.
- Promote optimal health outcomes through assessment, planning, implementation, and evaluation of patient care plans.
- Work collaboratively with a multidisciplinary healthcare team to develop tailored care plans.
- Perform discharge planning and acute care case management.
- Conduct basic nursing assessments and take vital signs.
- Maintain accurate documentation and reporting of patient care.
- Manage multiple priorities while ensuring high standard of patient care.
Requirements
- Years of Experience: 2 year(s)
- Charting System: Epic
- Travel Experience : Yes
- Current Registered Nurse (RN) license in the state of practice required.
- Must have 2 years Acute Care Case Management/Discharge Planning.
- Strong clinical knowledge in areas such as behavioral health, emergency medicine, primary care, hospital medicine, medical-surgical nursing, and emergency room protocols.
- Proficient in taking vital signs and performing basic nursing assessments.
- Excellent communication skills with the ability to work effectively within a team environment.
- Strong organizational skills with attention to detail in documentation and reporting.
- Ability to manage multiple priorities while maintaining a high standard of patient care.
