About the role

Key responsibilities & impact
  • Ensure continuity and quality of services by managing temporary and complex cases
  • Provide person-centered case management services to individuals with disabilities, mental health conditions, or terminal illness across multiple counties
  • Support new hires, stabilize cases, and advocate for individuals to achieve personal goals through community resources
  • Model the mission and values of Accord
  • Manage temporary cases and correct case work
  • Advocate for persons supported and assist with new County Transfers
  • Act as a leader on the team, offering shadow visits
  • Interact professionally with interdisciplinary team members
  • Meet face-to-face and remotely with individuals and their support teams to assess wellness
  • Monitor Medicaid eligibility status
  • Manage allocations and authorize services within the budget
  • Attend all required meetings and demonstrate professionalism
  • Complete documentation of all services in the electronic health record/database system

Requirements

What you’ll need
  • Bachelor's Degree in social work, nursing, psychology, sociology or a closely related field from an accredited four-year college or university; OR
  • A bachelor’s degree from an accredited four-year college or university in a field other than social work, nursing, psychology, sociology or a closed related field AND one year of experience in the delivery of social services to the target population as a supervised social worker in a public or private social services agency.
  • If you hold a Social Work degree, we expect you to be licensed by the Minnesota Board of Social Work, as required.
