Develop community support plans (goals) for each individual (typical caseload 40-45 individuals)
Assist individuals to access services, develop service agreements and documentation through case notes
Inform the person or legal guardian of service options that are a fit for their needs
Meet one-on-one with each individual at least twice a year to evaluate progress and adjust needs
Identify potential providers to guide individuals towards goals
Evaluate and monitor services identified in the plan
Follow case management rule 185
Comply with MN Health Care Programs related to delivery of waiver services (CADI, BI, EW, DD), complete MnChoices assessments and use state systems as required
Requirements
Meet ONE of the following criteria:
Bachelor's Degree in Human Services, Social Work, Psychology, Sociology, or a related field.
If you hold a Social Work degree, we expect you to be currently licensed as a Social Worker as required by the Minnesota Board of Social Work.
OR Bachelor's degree in any other field with at least one (1) year of experience working with individuals with a mental illness or disability
Good written and oral communication skills.
Excellent at working independently and in crisis situations.
Comfortable with high-volume paperwork.
Attention to detail and process oriented.
Able to pass a DHS (Department of Health Services) background check, valid driver’s license and clean driving record
Reliable transportation.
