Social Worker

• Assist our patients and their families/caregivers by assisting them with education and support as they cope with their recent diagnosis.
• Assess social and emotional factors related to the patient's illness, medical and nursing requirements in the home.
• Develop and implement a plan for home social work services, including education of patients, family members and care givers.
• Coordinate with other home health staff and community resources to provide an efficient integration of care.

Clinical Social Worker, MSW/LCSW – Home Health

General

• Provide personal care including, baths, back rubs, oral hygiene, shampoos, and changing bed linen
• Assist patients with dressing and undressing
• Provide skin care to prevent breakdown
• Assist the patient with toileting activities
• Keep patient’s living area clean and orderly, as appropriate
• Assist in feeding the patient, if necessary.
• Perform range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency).
• Assist patient in the self-administration of medication.
• Meet safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.).
• Report on patient’s condition and significant changes to the assigned nurse.

Certified Home Health Aide, CHHA

Therapist

• In partnership with the Case Manager you will provide skilled services to evaluate and address the needs of individuals in their homes, so they can live safely and productively in their home environment. These areas may include home safety medication management, wheelchair seating and positioning, pain management, falls prevention, management of chronic diseases and dementia family caregiver training.
• You will serve as an educator, patient advocate and a member of the interdisciplinary care team.

Occupational Therapist – Home Health

• In partnership with the Case Manager you will provide skilled services to evaluate and address the needs of individuals in their homes, so they can live safely and productively in their home environment.
• These areas may include home safety medication management, wheelchair seating and positioning, pain management, falls prevention, management of chronic diseases and dementia family caregiver training.
• You will serve as an educator, patient advocate and a member of the interdisciplinary care team

• Responsible for in the field oversight of clinicians who provide care and treatment to patients in their place of residence including performance evaluations and hiring.
• Handles incoming calls timely, and follows up as necessary to ensure appropriate actions are taken to meet the internal and external customer needs.
• Coordinates training and educational programs for nurses within the field to improve clinical outcomes and competencies.
• Assesses staff learning needs and formulates specific plans and goals for next evaluation period with employees, performs administrative call duties, and serves as a role model and resource for staff.
• Functions as the first point of contact for field clinicians to assist in patient care escalations etc.
• Coordinates at least bi-weekly Case Conferences with clinical team to discuss patients' progress and for revision of the care plan as needed to meet planned outcomes.
• Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist clinicians as appropriate.
• Reviews patient’s clinical diagnosis, medications, procedures and clinical course.
• Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing a plan of care.
