Therapist

• Perform an evaluation in the home and develop a care plan and work with PTA's to follow the care plan.
• Help patients with mobility, movement, and help in regaining their independence.
• Create detailed treatment and care plans for each patient.
• Communicate and coordinate with patients’ physicians to ensure a quality level of care.
• Evaluate and report patients’ progress to MD.

Physical Therapist Case Manager – Home Health

Manager

• Utilize your outstanding clinical skills as you create, implement and execute the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes.
• In this leadership role, you will have oversight of 2 Nurses who are responsible for executing your care plan.
• You will be responsible for conducting comprehensive patient assessments such as Start of Cares (SOCs), Evaluations, Resumption of Care and discharge planning.
• You will accomplish this with oversight of the assistance and teaching of patients’ and their family/caregivers with wound care, blood draws, tube feedings, Tracheostomy care, foley catheters, Congestive Heart Failure (CHF) management and medication education in the patients home.

Registered Nurse Case Manager, Level 2

• In partnership with the Case Manager you will provide skilled services to evaluate and address the needs of individuals in their homes, so they can live safely and productively in their home environment.
• These areas may include home safety medication management, wheelchair seating and positioning, pain management, falls prevention, management of chronic diseases and dementia family caregiver training.
• You will serve as an educator, patient advocate and a member of the interdisciplinary care team

Occupational Therapist

• Utilize your outstanding clinical skills as you create, implement and execute the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes.
• Perform a full assessment of the patient & family upon initial visit.
• Use assessment data to develop an advanced plan of care including rehabilitation.
• Work with patients and family one on one to achieve improvement and/or stability.
• Case manage patients care episode with interdisciplinary clinical team.
• Prepare clinical notes, orders and charting in a compliant time frame.
• Collaborate with physicians and agency staff to deliver the best care possible.
• Initiate and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols.

Registered Nurse Case Manager – Home Health

• Serve as an educator, patient advocate and member of interdisciplinary care team
• May be assigned a primary location but occasionally provide coverage outside primary territory for vacancies, leaves, higher patient census, or other client care needs

Occupational Therapist, Home Health

• In partnership with the Case Manager, provide skilled occupational therapy services in patients' homes
• Evaluate and address patient needs to promote safe, productive home living
• Provide home safety assessments and medication management support
• Address wheelchair seating and positioning, pain management, and falls prevention
• Manage chronic disease and dementia-related caregiver training
• Serve as educator, patient advocate, and member of the interdisciplinary care team
• Create flexible schedules and cover a targeted territory (Anticipated Territory: Milpitas Area)
• May provide coverage outside primary area to support vacancies, leaves, higher patient census, or client care needs
