About the role

Key responsibilities & impact
  • Provide direct support to people with intellectual and developmental disabilities
  • Promote community integration and inclusion
  • Assist with activities of daily living, including intimate and personal care
  • Teach daily and independent living skills and provide individual and group counseling
  • Provide clinical support for habilitative, behavioral, and psychiatric needs under the BIS’s supervision
  • Educate direct-care staff and program leadership on habilitative and Behavior Support Plans
  • Support clinical assessments and interventions and ensure consistent data collection
  • Provide direct feedback and lead the team on shift when supervisors are absent
  • Identify, facilitate, and schedule community inclusion activities; coordinate resources and document completion
  • Communicate and document supervision-level updates and changes
  • Identify and escalate health, behavioral, and clinical concerns
  • Implement individualized programming and Behavior Support Plans
  • Train direct-care staff using the EMOE training system and support development of OPWDD Core Competencies
  • Serve as lead trainer for weekend staff and support new staff
  • Participate in treatment-plan, habilitative-plan, life-plan, interdisciplinary-team, and clinical-team meetings
  • Gather data for behavioral and habilitative assessments and monitor progress toward goals
  • Maintain paperwork, monthly progress summaries, psychiatric/therapy notes, and other required documentation
  • Monitor day-to-day program operations, staff performance, and the wellness of people supported
  • Complete required trainings and certifications and comply with HIPAA and other regulations
  • Transport individuals when required by the assignment

Requirements

What you’ll need
  • Bachelor’s degree in psychology, social work, or closely related Human Services discipline and at least six months of relevant experience; or formal training including college coursework in relevant fields and at least two years of relevant experience; or at least four years of direct-care experience in a YAI program in good standing
  • Working knowledge of behavioral/psychiatric conditions and the interplay between I/DD and other conditions
  • Ability to gather, document, and communicate information accurately
  • Exceptional written and oral communication skills
  • Ability and willingness to lead, direct, and teach others
  • Must hold foundational certifications: SCIP-R, CPR issued by the American Red Cross or American Heart Association, and YAI-issued AMAP certification for residential assignments or Day Services Model Mentorship Training for day assignments
  • Newly hired employees must obtain and maintain required YAI certifications within six months of appointment
  • Must be at least 18 years old
  • Ability to meet essential physical demands, including walking, sitting, standing, bending, lifting/moving individuals, and lifting or moving up to 25 pounds occasionally
  • Ability and willingness to assist with ADLs and handle bodily fluids
  • Strong clinical skills, including assessing and analyzing information, compiling and aggregating data, and interpreting information
  • Ability to remain calm, act assertively, exercise judgment in stressful situations, and work in a fast-paced environment
  • Proficiency with computers, email, Workday, and related electronic systems
  • Ability to work full-time, including at least one regularly scheduled weekend day, holidays, inclement weather, emergencies, and beyond scheduled hours when needed
  • Ability to travel to regional trainings, meetings, and locations within the NYC metro area
  • Valid driver’s license and Van certification required for assignments involving transportation
  • Preferred: active pursuit of a Master’s degree and two or more years of I/DD experience
