About the role

Key responsibilities & impact
  • Provide individual supports and short-term case management to community members who are homeless or at risk of losing their housing
  • Facilitate community engagement and outreach
  • Create housing summits alongside community members to understand local needs, strengths, and opportunities
  • Outreach and engage residents who are homeless or at risk of homelessness
  • Support community members in accessing health and social services through a warm transfer process
  • Develop partnerships with service agencies to ensure efficient and timely access to services
  • Provide short-term case management and make referrals supporting housing retention
  • Compile reports on housing needs and community recommendations
  • Develop and maintain a directory of available housing, supports, and services
  • Explore and help develop a roommate matching program for community members
  • Work in Newmarket and across York Region

Requirements

What you’ll need
  • Undergraduate degree, preferably BSW/MSW
  • Minimum 1 year case management/short-term case management experience
  • Minimum 1 year related experience supporting people with housing needs
  • Knowledge and appreciation of housing issues in York Region
  • Knowledge of legislation and regulations, including the Residential Tenancies Act
  • Knowledge of eviction prevention strategies
  • Knowledge of community resources and demonstrated ability to search for applicable resources/information
  • Must be able to work in office and commute across York Region
  • Strong interpersonal skills, with an ability to work closely with clients and partner organizations
  • Problem-solving skills in identifying and referring individuals to resources
