About the role

Key responsibilities & impact
  • At Western Health, we’re redefining how care is delivered, moving beyond hospital walls to provide high-quality, patient-centred care where it matters most: at home.
  • You will lead a diverse, community-based service that directly impacts hospital flow, patient outcomes, and system sustainability, supporting patients to receive safe, effective care in their homes and residential aged care settings.
  • With oversight of approximately 37 FTE, you will drive performance, integration, and clinical excellence across multiple service streams, including: Residential In-Reach (RIR), Community Nursing, Central Access Unit, Liaison Nursing Team, Remote Patient Monitoring.
  • Lead a geographically dispersed workforce delivering complex care in community settings.
  • Drive integration across hospital and community interfaces, ensuring seamless patient journeys.
  • Optimise access and flow through effective intake, triage, and referral pathways.
  • Embed consistent, high-quality models of care across all service streams.
  • Build and sustain a high-performing, patient-centred team culture.
  • Partner with internal and external stakeholders to expand innovative models of care.
  • Translate strategy into operational delivery, aligning with Western Health’s vision for care closer to home.

Requirements

What you’ll need
  • Registered Nurse with current registration through Australian Health Practitioner Regulation Agency, with relevant postgraduate qualifications (or working towards)
  • Proven experience leading complex healthcare services, with accountability for performance, workforce and budget management
  • Strong clinical capability, with a commitment to safe, high-quality, person-centred care
  • Demonstrated ability to lead, influence and develop high-performing, multidisciplinary teams
  • Highly developed communication and interpersonal skills, with the ability to engage stakeholders and drive outcomes
  • Sound understanding of clinical governance, legislation and human resource processes
  • A systems thinker who can improve access, flow and integrated care pathways
  • Innovative, adaptable and able to lead and sustain change in a dynamic environment
  • Strong digital literacy and ability to work across multiple systems
  • Current Victorian Driver’s Licence
