About the role

Key responsibilities & impact

Provide nursing care to patients in their homes
Administer medications, wound care, and other treatments
Monitor patients’ conditions and report changes to the care team
Assist in developing personalized care plans
Educate patients and families on health management and self-care techniques
Collaborate with the healthcare team to ensure the highest quality care
Visit 5-6 patients on any given shift

Requirements

What you’ll need

Must be a licensed Occupational Therapist
Strong assessment and documentation skills
Ability to check email intermittently during the week
Willingness to travel throughout the service area
