Hospice Registered Nurse – Case Manager

About the role
Key responsibilities & impact- Provide skilled nursing care to hospice patients, focusing on pain management, symptom relief, and emotional support.
- Manage a caseload of patients, develop personalized care plans, and coordinate care with the hospice team.
- Educate patients and families about the hospice process, end-of-life care, and coping strategies.
- Assess patient needs regularly, report changes in condition, and adjust care plans accordingly.
- Offer emotional support to families and provide guidance throughout the hospice journey.
Requirements
What you’ll need- Graduate of NLN accredited school of nursing
- Current license to practice professional nursing as a Registered nurse in the state
- Bachelor’s Degree preferred
- Must have current Registered Nurse license in NC
- NC drivers license with an automobile that is insured in accordance with state or organization
- Current American Heart Association CPR certification
- Minimum of one years’ clinical experience required; hospice experience preferred
- Working knowledge of Word, E-mail, typing of 30 wpm
- Basic technical or medical knowledge of home health billing requirements
- Advanced customer service skills to respond with simple answers, etc
- Ability to work independently as well as effectively as part of an interdisciplinary team.
