About the role
Key responsibilities & impact- The ROOTS Hub Initiative 5 Lead reports to the ROOTS Hub Initiatives Manager.
- Responsible for supporting the implementation and operational oversight of Initiative 5 within NCDHHS’ defined region 3 comprised of several counties.
- Focus on advancing value-based payment readiness, healthcare sustainability, and innovative financing models for rural primary care providers and rural health systems.
- Collaborate with healthcare providers, payers, finance teams, implementation partners, and community stakeholders to support operational transformation, value-based care readiness, fiscal sustainability, and data-informed healthcare delivery improvements within rural communities.
- Serve as a strategic implementation partner to the ROOTS Hub Initiatives Manager and support operational planning, stakeholder engagement, reporting, governance activities, community engagement, and implementation monitoring related to Initiative 5 objectives and deliverables.
- Coordinate with region 3 lead anchors and tribal representatives where applicable and tailor NCRHTP initiatives and activities to meet region 3’s rural health needs and the NCRHTP performance objectives.
Requirements
What you’ll need- Bachelor’s degree in public health, healthcare administration, business administration, finance, or related field required.
- Master’s degree preferred.
- Minimum of two (2) years of experience project managing large and complex engagements.
- Minimum of two (2) years of experience coordinating across different types of stakeholders.
- Minimum of two (2) years of experience in an organization or field that demonstrates an understanding of the healthcare systems, Medicaid and/or Medicare payer systems, clinical and social needs on communities’ and/or populations’ access to health services.
- Minimum of two (2) years of experience serving or working on behalf of Medicaid and/or Medicare beneficiaries preferred.
- Population health experience and experience with physical health, behavioral health, I/DD and TBI populations preferred.
- Certifications in use of project management methodologies preferred.
- Certification in quality and process improvement preferred.
- Knowledge of value-based care, healthcare financing, rural healthcare systems, and population health principles.
- Strong project management and operational planning skills.
- Ability to coordinate across healthcare systems and stakeholder groups.
- Strong analytical, facilitation, and communication skills.
- Ability to interpret operational and performance data.
- Ability to manage multiple priorities and deadlines.
- Knowledge of Medicaid, Medicare, and healthcare reimbursement structures preferred.
- Proficiency in Microsoft Office applications.
