Key responsibilities & impact

Providing proactive intervention and coordination of care to eligible Vaya Health members
Identify and alleviate inappropriate levels of care or care gaps
Ensure appropriate assessment and services are delivered to members
Work with care teams to develop care planning and necessary support

Requirements

What you’ll need

Master’s degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area
Two years of experience working directly with individuals with BH conditions
Two years of prior Long-term Services and Supports and/or Home Community Based Services coordination, care delivery monitoring and care management experience
Valid licensure required: Acceptable licenses include Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), Licensed Clinical Mental Health Counselor (LCMHC), etc.
Experience working directly with individuals with I/DD or TBI preferred
