About the role
Key responsibilities & impact- Demonstrate the UVA Professional Practice Model through professional nursing practice, quality achievement, lifelong learning, empowered leadership, innovation, and expert caring
- Manage complex care and high-risk patient populations, alleviating barriers to care
- Triage new patient referrals and assess patient needs, goals, and preferences, integrating them into the care plan
- Coordinate multidisciplinary care and expedite timely access to treatments, including diagnostic testing, surgery, and follow-up care
- Serve as a clinical advocate, educator, and liaison between patients, families, and healthcare providers
- Provide culturally competent education on diagnosis, treatment, side effects, and posttreatment care
- Facilitate communication between care teams and support participation in therapeutic clinical trials
- Work with multidisciplinary teams to identify programmatic and quality initiatives, tracking and monitoring outcomes related to the care pathway
Requirements
What you’ll need- Bachelor of Science in Nursing from an accredited nursing program
- 1 year of registered nursing experience required
- 3 years of related and relevant registered nursing experience strongly preferred
- Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia
- American Heart Association Health Care Provider BLS certification required
- Specialty Board Certification strongly preferred
- Ability to stand for prolonged periods
- Ability to frequently travel
- Ability to bend and stoop
- Proficient communicative, auditory, and visual skills
- Attention to detail and ability to write legibly
- Ability to lift, push, or pull 20–50 lbs.
- Ability to work around chemicals, blood/body fluids, and infectious diseases
