About the role
Key responsibilities & impact- Collaborate with physicians, APPs, palliative care social workers, and chaplains to provide timely access and care to patients with palliative care needs
- Provide all necessary acute care services to patients at M Health Fairview
- Provide timely consultations to requesting clinicians in different settings
- Ensure proper care, diagnosis, and treatment as necessary
- Communicate with patients and families about diagnoses, diagnostic or therapeutic treatments, procedures, prognosis, and potential or expected outcomes
- Communicate with primary and/or referring physicians
Requirements
What you’ll need- Graduate of an accredited ACGME Hospice and Palliative Medicine training program
- Board eligible/certified in Hospice and Palliative Medicine
- Licensed or ability to acquire medical license and DEA in the State of Minnesota
- Meet threshold criteria/qualifications for credentialing and privileges
