About the role

Key responsibilities & impact
  • Deliver care for patients on the University of Minnesota Heart Care team at M Health Fairview Heart Care Clinics in Maplewood and Woodbury and at St. John’s Hospital in Maplewood
  • Coordinate and provide care for adults with cardiac diseases throughout evaluation and treatment
  • Provide outpatient management for adults with heart failure of various etiologies
  • Potentially round and provide inpatient management of heart failure patients
  • Provide patient and family education related to heart failure and general cardiology
  • Provide clinic coverage in heart failure and general cardiology
  • Perform clinic follow-up visits and potentially assist with inpatient consults
  • Manage patient care through appropriate use of available resources
  • Adjust care or service approaches to reflect the developmental level of the population served
  • Use the fully integrated EPIC Electronic Medical Record
  • Contribute to improved patient satisfaction, quality of care, access, and coordinated services
  • Collaborate with a multidisciplinary team of cardiologists and advanced practice providers

Requirements

What you’ll need
  • Graduate of an accredited Physician Assistant or formal Adult Nurse Practitioner training program
  • Eligible for or certified through NCCPA, ANCC, or AANP
  • Licensed or eligible to acquire a license and prescriptive authority through the Minnesota Board of Medical Practice for PAs
  • Licensed or eligible to acquire a Minnesota APRN license and prescriptive authority through the Minnesota Board of Nursing for NPs
  • Able to meet threshold criteria and qualifications for credentialing and privileges
  • Heart failure experience is an advantage
  • Certified Heart Failure Nurse certification through AAHFN, or eligible for certification, is an advantage
  • Commitment to exemplary patient care
  • Ability to provide care for adults with cardiac diseases
  • Ability to use EPIC Electronic Medical Record
  • Must receive an annual flu vaccination unless a reasonable accommodation or exemption is granted
