About the role
Key responsibilities & impact- Perform comprehensive member assessments, including face-to-face and in-home visits
- Develop and implement individualized care plans with members, caregivers, physicians, and support networks
- Monitor care plans, document interventions, and adjust plans as needed
- Promote integration of behavioral health, LTSS, and community resources
- Evaluate benefits and advise on funding sources
- Facilitate interdisciplinary care team meetings and collaborate with team members
- Use motivational interviewing to educate and support members
- Identify barriers to care and address psychosocial, financial, and medical concerns
- Develop prevention plans for critical incidents to protect member health and safety
- Maintain accurate documentation in electronic systems and comply with standards
- Travel locally 25–40% for member visits
- Perform other duties as assigned
Requirements
What you’ll need- Active, unrestricted Oregon Registered Nurse (BSN or MSN) license
- Qualifications meeting eligibility requirements to sit for the Certified Case Manager (CCM) examination
- Minimum of two years of healthcare experience
- At least one year supporting individuals with disabilities or chronic conditions in Long-Term Services and Supports (LTSS)
- At least one year of experience in care management or a medical and/or behavioral health setting
- Valid driver’s license, reliable transportation, and current automobile insurance
- Proficiency in Microsoft Office Suite
- Ability to navigate electronic health records and care management systems
- Strong knowledge of Oregon community resources
- Experience working with diverse populations
- Strong time management, multitasking, and problem-solving skills
- CCM certification required within 18 months of hire
- Knowledge of Medicaid and Medicare programs, including waiver services
- Familiarity with healthcare regulatory and compliance standards
- Experience providing culturally competent care to diverse and underserved populations
- Accurate electronic documentation experience
- Bilingual or additional language skills are a plus
