About the role

Key responsibilities & impact
  • Support disease management through proactive patient monitoring, assessment, education, and follow-up
  • Empower patients and families in self-care practices and motivational and behavioral change modalities
  • Provide competency-based education to patients, staff, and providers
  • Maintain care management databases
  • Provide education and preventive health measures and help patients with chronic disease or conditions set behavior-change goals
  • Participate in controlling cost-effective services through appropriate staffing, equipment, supplies, and patient-care management
  • Incorporate restorative-care principles into daily direct care
  • Evaluate, develop, and implement clinical program components to optimize patient-care strategies and goals
  • Conduct interviews and assessments and use information from residents/families to plan, implement, and evaluate care
  • Provide timely internal and external customer service professionally and respectfully

Requirements

What you’ll need
  • Must be licensed as an RN for two (2) years
  • Valid RN license
  • RN experience in clinical nursing with chronic disease, utilization review, and/or quality management preferred
  • Knowledge of evidence-based clinical guidelines across a wide variety of conditions and age groups
  • Knowledge of care management and resource/utilization management
  • Skill in using personal computers and related software applications
  • Ability to manage multiple priorities under time constraints
  • Ability to analyze and solve problems
  • Understanding of cost and quality issues
  • Verbal and written communication skills
  • Interpersonal skills to interact with a wide range of constituencies
  • Decision-making skills
  • Ability to occasionally travel to offsite locations
  • Ability to meet physical demands including occasional lifting and carrying up to 25 pounds, frequent standing, sitting, reaching, and walking
