About the role
Key responsibilities & impact- Perform in-home assessments, clinical monitoring, caregiver training and oversight, and skills-based instruction depending on the client and program assignment
- Work across three programs — skilled home health, In-Home Support Services (IHSS), and Independent Living Skills Training (ILST)
- Provide clinical autonomy in the field without fixed shift commitments, mandatory overtime, or on-call requirements
Requirements
What you’ll need- Active, unrestricted Colorado RN license (or compact multistate privilege)
- Minimum 1 year clinical nursing experience
- Current BLS/CPR certification
- Reliable transportation, valid CO driver's license, and current auto insurance
- Ability to travel across the Greeley and/or Northglenn service areas
- Ability to pass background screening and required health clearances
- Home health, hospice, case management, or community health experience (strongly preferred)
- Experience with brain injury, neuro rehab, developmental disability, or geriatric populations (strongly preferred)
- Comfort with electronic documentation (strongly preferred)
- Bilingual English/Spanish (strongly preferred)
