About the role

Key responsibilities & impact

Perform in-home assessments, clinical monitoring, caregiver training and oversight, and skills-based instruction depending on the client and program assignment
Work across three programs — skilled home health, In-Home Support Services (IHSS), and Independent Living Skills Training (ILST)
Provide clinical autonomy in the field without fixed shift commitments, mandatory overtime, or on-call requirements

Requirements

What you’ll need

Active, unrestricted Colorado RN license (or compact multistate privilege)
Minimum 1 year clinical nursing experience
Current BLS/CPR certification
Reliable transportation, valid CO driver's license, and current auto insurance
Ability to travel across the Greeley and/or Northglenn service areas
Ability to pass background screening and required health clearances
Home health, hospice, case management, or community health experience (strongly preferred)
Experience with brain injury, neuro rehab, developmental disability, or geriatric populations (strongly preferred)
Comfort with electronic documentation (strongly preferred)
Bilingual English/Spanish (strongly preferred)
