About the role
Key responsibilities & impact- Care for patients throughout Indiana via telehealth.
- Evaluate and treat patients through personalized hormone replacement therapy (HRT), medical weight management, and preventive healthcare.
- Engage in a collaborative telemedicine environment and build long-term patient relationships.
Requirements
What you’ll need- Strong clinical assessment and medical decision-making skills.
- Excellent patient communication and relationship-building abilities.
- Active Indiana Family Nurse Practitioner (FNP) license with Indiana APRN licensure (Family specialty only) or an active Indiana Physician Assistant license under your primary taxonomy NPI.
- Current Indiana DEA and CSR registrations (required at the time of application; no company reimbursement is provided).
- Current National Provider Identifier (NPI).
- Current board certification applicable to your profession.
- NPI number and board certification information must be disclosed during the application process to be considered.
- No current or pending malpractice claims or disciplinary actions.
- Experience or a strong interest in hormone replacement therapy, preventive medicine, men's health, women's health, and medical weight management is preferred.
- Bilingual English/Spanish is a plus.
