Requirements

What you’ll need

Active Texas Family Nurse Practitioner (FNP) license with Texas APRN licensure (Family specialty only) or an active Texas Physician Assistant license under your primary taxonomy NPI.
Current Texas DEA registration (required at the time of application; no company reimbursement is provided).
Current National Provider Identifier (NPI) number.
Current board certification applicable to your profession.
No current or pending malpractice lawsuits.
Experience or strong interest in hormone replacement therapy, women's health, preventive medicine, or medical weight management is preferred.
Bilingual English/Spanish is highly preferred.
