Key responsibilities & impact

Develop, implement, and evaluate population health initiatives centered around medication management, adherence, and optimizing therapeutic outcomes.
Utilize health data analytics to identify trends, gaps in care, and opportunities for intervention.
Collaborate with healthcare providers to ensure appropriate medication selection, dosing, and monitoring.
Design and implement programs to educate patients about their medications.
Work closely with healthcare teams to coordinate care plans and interventions.
Participate in quality improvement projects aimed at enhancing medication-related outcomes.
Provide patient care in an appointment-based model for both dispensing and non-dispensing patients.
Stay updated with pharmaceutical regulations, ensuring compliance.
Responsible for hiring, training, and managing a team of Pharmacy Care Coordinators.

Requirements

What you’ll need

Doctor of Pharmacy (PharmD) degree from an accredited institution.
Active pharmacist licensure in the state or region of practice.
Advanced training or certification in population health management or a related field (preferred).
Strong analytical and data interpretation skills.
Experience in pharmacy practice, preferably in a population health or managed care setting.
Excellent communication, collaboration, and leadership abilities.
Knowledge of healthcare regulations, policies, and quality improvement methodologies.
