About the role

  • Evaluates, plans, directs and administers individualized therapeutic treatment programs aimed at the restoration or improvement of function, or the prevention of disability
  • Writes evaluations, progress notes/reports, discharge summaries and maintains patient files
  • Counsels patients and/or family members regarding Rehabilitation diagnosis, plan of care and prognosis
  • Maintains current knowledge of regulatory/legislative policies as they apply to the profession of Physical Therapy and adheres to same

Requirements

  • Graduate of Accredited Physical Therapy Program
  • Current NJ State License in Physical Therapy
  • CPR certification within first 3 months of hire
  • FIM certification to be taken following employment and training at Wayne (Uniform Data System for Medical Rehab-UDSmr)
  • Valid driver’s license and comply with Qualifications to Drive a System Vehicle and SJHS Fleet Management & Safety Program (EIP employees only)
  • BLS, ACLS and PALS certifications must be granted through the American Heart Association (effective January 2019)
