About the role
Key responsibilities & impact- Coordinate and manage care services for patients
- Ensure high-quality, timely, and coordinated care
- Document care in compliance with International Classification of Diseases (ICD) coding standards
- Work closely with healthcare providers to develop care plans
- Coordinate referrals and ensure patient progress
- Review medical records
- Identify coding discrepancies
- Provide education and guidance to internal and external staff on ICD coding guidelines
- Ensure member needs are addressed and refer members to appropriate service providers
- Administer procedures for verifying member identity, membership status, and entitlement to services
- Maintain current and accurate membership databases and records
- Suggest new service offerings or revisions to existing programs based on member feedback or gaps in coverage
- Participate in member retention or renewal efforts
- Support the achievement of SCAN’s Vision and Goals
Requirements
What you’ll need- High School Diploma or equivalent experience
- 0-3+ years of related experience
- Performs work under minimal supervision
- Handles complex issues and problems, referring only the most complex issues to higher-level staff
- Comprehensive knowledge of subject matter
- Provides leadership, coaching, and/or mentoring to a subordinate group
- May act as a lead or first-level supervisor
- PC skills including Email, Word, Excel, and PowerPoint
- Basic technical skills for the functional area
- Basic problem-solving skills
- Good communication and interpersonal skills
