About the role
Key responsibilities & impact- Provide long-term care management to frail, economically disadvantaged, culturally diverse older adults, disabled adults and caregivers
- Assess new clients and determine appropriateness for the MSSP program, certify nursing home eligibility
- Conduct assessments and reassessments for identification of client’s medical, physical, and psychological needs
- Ensure client’s needs are met through written care plan approved by the program supervisor
- Provide ongoing health education to non-clinical staff members
- Present cases at weekly team case conferences
- Discuss during case conference complex health care issues for teaching purposes
- Maintain monthly documentation of clients and clinical documentation of relevant contacts in a timely manner
- Develop and implement care plans for assigned caseload
- Participate regularly in Quality Assurance chart reviews
Requirements
What you’ll need- Nursing degree required
- Bachelor's degree in Nursing (BSN) preferred
- Registered Nurse required; Current and active California RN license in good standing
- 3+ years experience with health assessments, level of care criteria, care planning, and case documentation
- Experience working with older adults and conducting home visits highly desirable
- Ability to travel within program’s catchment area and visits to client homes is required
- Knowledge of environmental concerns of older adults
- Ability to perform home assessments, telephone monitoring, and travel to client homes throughout Los Angeles County
- Ability to work as a part of a team, while at the same time performing job independently
- Strong organizational, verbal, and written communication skills
- Ability to work well in a fast-paced and dynamic environment
- Demonstrated customer focus and experience working in a case management setting
- BILINGUAL- Must be bilingual in English/Korean.
